I think there are a few problems with the ToDo-List.

The list doesn't refresh when I click the 'Refresh'-button, it looks like the button does nothing.
The list also doesn't show all Todos of my project if I change the file or save it.
It looks to me like it looks only in the open files.

The option for Auto-Refresh is checked.

I think it should be possible to choose if the plugin looks only in the open files, or in the project files.

I can confirm that only opened Project fieles are parsed for ToDo in All Project FIles Mode
